On my server when ices is playing as an "autodj" , I've got ices configured to play an intro file on connect.

This file plays 1 or 2 beats of the intro (1 second long maybe) and then it "hiccups" or goes silent, and then the rest of the file plays fine.

When a live source client is connected to the server, I don't have an intro file configured to play... and the same thing happens, but with the live source audio.

Any idea what may be causing this. I'm using the default out of the box icecast.xml file only changed the domain name and passwords.

Are you saying the data stream drops to 0 bytes per second for a while or are you saying the audio stops playback. If the former doesn't happen then you need to check what data is being sent and how the player is reacting to it. using wget/curl should be enough to check the data rate.

Upon further investigation, this happen 90% of the time with iTunes on Mac OS X. 10% of the time, the intro file plays smoothly as expected with iTunes.

I cannot reproduce the issue with either Quicktime player or Real Player on Mac OS X.

Any idea why this happens only with iTunes?

the data rate looks fine here and the content seems valid, vlc plays it here so I don't think there is anything wrong with it.
